Winter Camp is only days away! Can you believe it?!

  1. Pre-Arrival Letters:

    • For parents, please find their pre-arrival letter here.

  2. Packing List: please find the list provided here.

  3. Required Forms: Ensure that all students and adult volunteers complete their required forms online. These forms can be found in the attendees' online accounts. Forms are not printed out, except for those campers bringing insulin or EpiPens. Printable forms for these cases are available on the Forest Home website under the "Forms" tab and the "First Aid" label.

 The required online forms include:

    • Medical Form (all attendees, including immunization dates or Personal Beliefs Affidavit)

    • Additional printed forms may be required for campers with more severe special medical needs.

If you have questions regarding this, please contact our First Aid Team at 909-389-4326. Adults need to complete the Adult Medical Form AND a Voluntary Disclosure form.

Note: If a camper's medical form is not completed by the time of arrival, a parent/legal guardian will be contacted to pick up their child by midnight.

 Medications and First Aid:

    • Prior to Arrival:

      • All medications must comply with the "sending medications to camp" form (pharmacy labeled, in original bottles, not loose pills, etc.).

      • Medications should be placed in a zip-lock bag labeled with the camper's name and church name.

      • All medications for campers and adults housed with campers must be collected before departing for camp.

      • Health screening and temperature checks will be taken upon check at LJPres for campers. This is a requirement made by Forest Home.
